This is the FOIA-released redacted Navy Investigation Report of the grounding of USS Connecticut (SSN 22) on October 2 2021 with an uncharted seamount while operating in a poorly surveyed area in international waters. There were 11 crew members injured (2 significant and 9 minor). The propulsion plant was not affected. The conclusion was that the event was avoidable. 50 crew members were identified as needing mental health treatment during the transit to port.
